Your Right to Your Protected Health Information
As a patient of LLUMC-Murrieta, you have the right to access and obtain copies of your protected health information (PHI).
To request a copy of your medical files please contact Health Information Management at 951-290-4510.
When requesting your records you must complete this form authorizing us to disclose your protected health information:
Different Ways to Receive Your Medical Record
- Paper Copy - $0.25 per page
- Fax - We cannot safely fax your records to you and preserve your privacy
- Email - We can send you an encrypted email with your records for no charge

Frequently Asked Questions
Is there a charge for medical record copies?
If you wish to have paper copies of your medical records there is a 0.25 per page charge.
Do you accept checks?
Yes, we accept personal checks, cashier’s checks, and money orders. The check should be payable to LLUMC-Murrieta.
Do you accept credit cards/debit cards?
No, at this time we cannot accept credit cards or debit cards.
How long does it take to fulfill my request?
We will fulfill your request within 15 calendar days upon receipt of your request.
Will you fax records to my home or business?
Due to confidentiality reasons we cannot fax medical records to your home or business.
Will you email my medical records?
Yes, we can provide medical records via encrypted email at no charge.
Will I be able to pick up my medical records?
Yes, after we have received and processed your request we will notify you that your records are ready to be picked up. You will need to bring a valid ID with you before medical records can be released.
May my spouse request and receive copies of my medical records?
To better protect your privacy another person may not receive copies of your medical records without a written consent that authorizes the medical center to provide copies to him or her.
How may I obtain a copy of my x-ray images?
We will provide the first set of x-ray images to you at no charge. For any additional copies there will be a 10 dollar charge.
